Course Fee:  No tuition or out of county fees for members or prospective members of Volunteer First Aid Squads who submit a signed EMT Training Fund Certificate of Eligibility Form (Below) with this application.  Otherwise enclose indicated tuition and out of county fees.


Note:  Eligibility Forms Do Not Include the EMT Textbook, which can be purchased at the EMS Training Center at the beginning of the course.
